Job Description:
The Nurse Educator serves in a patient support role by working with the intake department to facilitate timely and efficient discharges through the proper vetting of a patient’s suitability for home infusion and/or respiratory care and providing necessary education to allow timely/efficient discharges to occur. The Nurse Educator does this in collaboration with physician offices, hospital case managers, discharge planners and social work staff at area hospitals and Skilled Nursing Facilities. This person serves as a patient care advocate and facilitates the transition to post-hospitalization/facility care.
The Nurse Educator will travel as needed to various facilities as needed as patients are identified as potential home infusion and/or respiratory patients.
